Supporting Families Through Long Hospital Stays

A long hospital stay brings costs that add up quickly, whether you are across the country or across town. We are honored to provide grants to eligible families so your child can always remain your focus.

Details

• A one-time grant of $1,000 is available for eligible families
• Funds may be used in the way that best serves the family during the hospital stay
• Funding payments may only be made to the guardian of the child with a trisomy diagnosis
• Families who have received a Travel For Care grant may also receive an Extended Stay grant. This may apply, for example, to families who spend time in a local hospital and then transfer their care elsewhere

Eligibility

• Child's hospital stay must last 7 days or longer, regardless of distance from home
• Applicant must be the guardian of the child with Trisomy 18 or Trisomy 13
• Live in the U.S.
• The maximum total of all grants a family may receive in any rolling 12-month period is $2,500

Application Checklist

• Applicant must provide Driver’s License and photo of a utility bill at current mailing address if different than address on license
• Medical Documentation:
• Provide proof of trisomy diagnosis on official letterhead signed by provider, or from medical chart, with child or guardian’s name, date of birth, diagnosis, and treating facility or diagnosing entity (lab corp) visible.
• Provide documentation of the date of admission, such as a screenshot from your patient portal or a signed doctor's note
• If approved, applicant must provide bank account information for ACH payment, or have a physical check sent to primary address
